Accountable To: Goodwill Healthcare Services Limited (GHS CARE) Agency Manager/Director, Head Chef / Catering Manager / Care Home Manager

Contract Type: Temporary, Agency, Flexible Shifts, Zero-Hour Contract

Hours: Days, Evenings, Weekends, Nights and Bank Holidays as required

Location:

Residential Homes, Nursing Homes, Dementia Services, Hospitals, Rehabilitation Units, Supported Living Services, and Community Healthcare Settings across Middlesbrough, Newcastle upon Tyne, Sunderland, Darlington, Redcar, Stockton-on-Tees, Gateshead, Durham, Leeds, and surrounding areas within the Central North East Region, United Kingdom.


JOB PURPOSE

As a Registered Nurse (RGN, RMN or RNLD), you will be responsible for delivering safe, effective, evidence-based, and person-centred nursing care while maintaining the highest standards of clinical practice and professional conduct.

You will assess, plan, implement, and evaluate care for patients, residents, and service users while working collaboratively with multidisciplinary teams to achieve positive health outcomes. The post holder may take charge of shifts when required and provide leadership, supervision, and clinical guidance to Healthcare Assistants, Senior Care Assistants, Students, and Junior Nurses.

All duties must be undertaken in accordance with the Nursing and Midwifery Council (NMC) Code, organisational policies, safeguarding requirements, and relevant legislation.


KEY RESPONSIBILITIES

Clinical Care and Nursing Practice

  1. Deliver high-quality, person-centred nursing care in accordance with clinical standards, best practice guidelines, and care plans.

  2. Undertake comprehensive nursing assessments and contribute to the development, implementation, and evaluation of care plans.

  3. Administer medications, controlled drugs, treatments, and clinical interventions in accordance with NMC standards and organisational policies.

  4. Monitor, assess, and document vital signs, observations, clinical conditions, and changes in patient wellbeing.

  5. Perform clinical procedures including wound care, catheter care, PEG care, venepuncture (where competent), blood glucose monitoring, and other nursing interventions.

  6. Support residents and patients with Activities of Daily Living (ADLs) where required.

  7. Promote dignity, privacy, choice, independence, and person-centred care at all times.

  8. Provide compassionate end-of-life and palliative care in accordance with individual wishes and care plans.

  9. Support individuals living with dementia, mental health conditions, learning disabilities, and complex healthcare needs.


Medication Management

  1. Safely administer, record, and monitor medications in accordance with legislation and organisational policies.

  2. Complete medication administration records (MAR charts) accurately.

  3. Monitor medication effectiveness and adverse reactions.

  4. Report medication errors, discrepancies, or concerns immediately through appropriate procedures.

  5. Ensure safe storage, handling, and disposal of medications.


Assessment, Monitoring and Care Planning

  1. Assess patients' physical, psychological, emotional, and social needs.

  2. Develop and review person-centred care plans and risk assessments.

  3. Evaluate care outcomes and amend care plans where appropriate.

  4. Identify deterioration in health and escalate concerns promptly.

  5. Assist in diagnostic procedures and interpret clinical information to support care planning.


Record Keeping and Documentation

  1. Maintain accurate, timely, and comprehensive nursing records.

  2. Document all care, interventions, assessments, incidents, and communications appropriately.

  3. Ensure compliance with professional, legal, and regulatory documentation standards.

  4. Adhere to the principle:
    "If it is not documented, it has not been done."

  5. Complete incident reports, accident reports, safeguarding referrals, and clinical documentation as required.


Leadership and Supervision

  1. Take charge of shifts when required and ensure safe staffing and service delivery.

  2. Provide clinical leadership and support to Healthcare Assistants, Senior Carers, and Junior Staff.

  3. Supervise delegated tasks and ensure care is delivered safely and effectively.

  4. Support the induction, training, and development of colleagues.

  5. Act as a positive role model promoting professionalism and high standards of care.


Communication and Multidisciplinary Working

  1. Communicate effectively with patients, families, colleagues, and healthcare professionals.

  2. Participate in multidisciplinary team (MDT) meetings and reviews.

  3. Liaise with GPs, Pharmacists, Community Nurses, Hospitals, Mental Health Teams, Social Workers, and Allied Health Professionals.

  4. Provide clear verbal and written handovers.

  5. Respond appropriately to telephone enquiries and communications.


Infection Prevention and Control

  1. Implement infection prevention and control measures at all times.

  2. Follow current infection control policies, guidance, and best practice.

  3. Ensure appropriate use of Personal Protective Equipment (PPE).

  4. Report outbreaks, infections, and infection control concerns promptly.


Health, Safety and Risk Management

  1. Maintain a safe environment for patients, residents, visitors, and colleagues.

  2. Follow all Health and Safety policies and procedures.

  3. Complete risk assessments and implement risk management strategies.

  4. Report incidents, accidents, hazards, near misses, and safeguarding concerns.

  5. Participate in fire safety procedures and emergency response plans.

  6. Promote safe moving and handling practices.


Safeguarding

  1. Safeguard vulnerable adults and children in accordance with legislation and organisational policy.

  2. Recognise and report signs of abuse, neglect, exploitation, or harm.

  3. Maintain professional accountability for safeguarding responsibilities.

  1. Maintain strict confidentiality regarding patients, residents, service users, and organisational information.

  3. Only share information appropriately and on a need-to-know basis.


Resource Management

  1. Ensure effective and efficient use of clinical resources and equipment.

  2. Maintain accountability when handling service users' valuables and personal property.

  3. Report damaged equipment and shortages promptly.

  4. Promote cost-effective use of resources without compromising patient care.


REQUIREMENTS

Essential

  • Active NMC Registration (RGN, RMN, or RNLD).

  • Minimum 6 months nursing experience within a clinical, care home, hospital, or healthcare setting.

  • Medication Administration Competency.

  • Excellent clinical assessment and decision-making skills.

  • Strong communication and record-keeping abilities.

  • Right to Work in the UK.

  • Ability to work independently and as part of a multidisciplinary team.

Desirable

  • Driver with access to own vehicle.

  • Active DBS on the Update Service.

  • Experience within nursing homes, dementia care, mental health, learning disability, rehabilitation, or hospital settings.

  • Tracheostomy, PEG, Catheter, or Complex Care experience.

  • Leadership or Nurse-in-Charge experience.
